Omnihealth gladly welcomes insured and uninsured patients and new patients are currently being accepted. To become a patient, simply follow these easy steps:

1. Call our office at 480.464.4705 to schedule an appointment

2. Download and complete the New Patient forms and bring them with you. Or, on the day of your appointment or please arrive 20 minutes early to complete your forms.

3. Walk-in patients welcome, you can complete all necessary documents upon arrival

Omnihealth accepts the following insurance plans:

All Plans

  • Aetna
  • Health Net
  • Humana
  • Arizona Foundation for Medical Care
  • AARP Medicare Complete/Lifeprint
  • Scan
  • Cofinity
  • Medicare Part B
  • Most Medicare Supplement Plans
  • *PPO plans from United, BCBS, Cigna, and others that have out-of-network benefits (for Medicare plans, see below)

Commercial/Medicare Advantage Plan

  • All plans from the above payers
  • Most non-HMO Advantage Plans (usually PPO) from United, BCBS, Cigna, and others.

* Any required lab work or imaging completed outside of Omnihealth is additional.
